I confirm that I have read and understand all Sherry Martin Dance Information, Pricing & Policies located on this website for the 2023-24 dance season (click HERE to read). Students, family and staff assume personal responsibility for participation in the dance program. I understand that Sherry Martin Dance is not responsible for any injuries sustained prior to the beginning of classes. I recognize that my child's participation may expose him or her to the risk of injury or harm. I accept the risk and hereby release Sherry Martin Dance and all employees from all liability from personal injury, illness or property damage incurred during instruction or performances. I certify that my child is in good health and capable of participating in all activities and classes. Sherry Martin Dance is not responsible for any lost, stolen or damaged items. If my account is in arrears for more than 30 days, my child's participation will be in jeopardy regarding regularly scheduled dance classes and any performances (including competition). Photographs that are taken of my child may be used on the Sherry Martin Dance website or for advertising purposes.
